CLINICAL REVIEW Statin induced myopathy Sivakumar Sathasivam, Bryan Lecky Since their introduction for the treatment of hyper-cholesterolaemia in 1987, the use of statins has grown to over 100 million prescriptions per year.1 About 1.5-3% of statin users in randomised controlled trials The body of evidence regarding the use of genetic testing to assess the risk of statin-induced myopathy is sparse and of low quality. In particular, studies that evaluate the clinical validity and clinical utility of genetic testing for statin-induced myopathy are lacking.

Statin-associated autoimmune myopathy (SAAM) is a very rare form of muscle damage caused by the immune system in people who take statin medications. The exact cause is unclear. A combination of consistent findings on physical examination, the presence of anti HMG-CoA reductase antibodies in a person with myopathy, evidence of muscle breakdown, and muscle biopsy diagnose SAAM. ABSTRACT Statins up-regulate expression of 3-hydroxy-3-methylglutaryl coenzyme A reductase (HMGCR), the rate-limiting enzyme in cholesterol synthesis and the major target of autoantibodies in statin-associated immune-mediated necrotizing myopathy. As muscle cells regenerate, they express high levels of HMGCR, which may sustain the immune response even after statin therapy is stopped. Statin induced myotoxicity may cause an asymptomatic rise in creatine kinase concentration, myalgia, myositis, rhabdomyolysis and necrotizing autoimmune myopathy. • Risk factors of statin induced myotoxicity include patient-related risk factors, statin-related risk factors and risk factors related to statin–drug interactions. •
